Hence the clinician should have all the necessary relevant information in making decisions. For instance, when providing care to a patient, the patient’s past medical records are essential in helping the clinician decide on the patient’s diagnosis, maintenance, and treatment. In order to improve the patient’s diagnosis, it is necessary to consider any differential diagnosis and critically examine the symptoms and signs that may indicate a more severe illness.
