You have been promoted to nurse manager of your unit. You realize that to function effectively, you must have a better understanding of your organization’s structure and, specifically, the chain of command.

- Give a brief description of the organization of the healthcare structure used in your facility.
- Describe the chain of command used in your organization and identify the people you report to and those who report to you.
- Consider the following and discuss how the chain of command is used to implement change.
- Based on the organization chart, what leadership structure is used in the organization—hierarchical or flat? How does this affect implementing change?
- Does this organization use a shared governance model for nursing?

Answer preview
Even though this hierarchical structure has its own set of advantages, it also has a significant downside, especially when it comes to change management and implementation. It is very hard to implement any changes without the input of senior management, who retain the decision-making capabilities within the organization (Kimchi, 2015). The lack of devolved or delegated decision-making means that senior management must first approve everything before it gets implemented, irrespective of how small or big the proposed change is. Such a process is prolonged and tedious, aspects that make it quite difficult for nurses, who interact with patients most of the time, and as such, are familiar with the various issues affecting the quality and safety of the care they receive, to not only propose pertinent changes but also implement them. This lack of a shared model for governance and decision-making adversely affects the change management and implementation process at Baptist Health South Florida (Kimchi, 2015).
