Scenario
Your hospital received $4,000,000 in revenue this past year. Medicare withholds 2.6% of this amount ($) until the value-based purchasing scores are in. If the scores are good this year, the hospital will earn back this money. If scores are below the national average, the hospital can lose this money. Twenty five percent (25%) of this score is based upon the Community Engagement patient satisfaction score (HCAHPS). Last year’s patient satisfaction scores (HCAHPS) in the following areas were below the national average, which cost your hospital $398,000 in revenue.
The hospital director of Quality Improvement has asked you to participate in developing a nursing action plan for your facility.

1.Communication about medication is the issue selected for this class. Healthcare professionals should educate the patients about the medications and their side effects before discharging them. HCAHPS was established to measure the healthcare consumers’ perceptions of the healthcare organization’s experience. HCAHPS aims at enhancing the patient’s choice, promoting the healthcare practitioner’s responsibility, and offering performance incentives based on patient feedback. In simple terms, this item measures the satisfaction of the patients based on the education and administration received from a given healthcare facility, thus helping CMA star ranking and reimbursement. Healthcare organizations must offer quality health services to the patients and, at the same, train their healthcare professionals to educate the patients on the medication and side effects to receive higher scores and reimbursements. Ineffective education to the patients attracts lower scores as patients did not receive the patients satisfactorily. Lower patient satisfaction may hurt the healthcare organization’s financial position as Medicare is more to withhold a proportion of reimbursement, thus interfering with its revenue flow. Ineffective communication occurs as a result of a combination of factors, including the nurse’s burnout, wrong patient/data communications barrier, and workload pressure, among others. Ineffective communication increases medication errors, thus lowering quality scores for the health facility.
