Case Study #3: Insomnia and Sleep Disorders

S.H., age 47, reports difficulty falling asleep and staying asleep. These problems have been ongoing for many years, but she has never mentioned them to her health care provider. She has generally “lived with it” and self-treated the problem with OTC Tylenol PM. Currently, she is also experiencing perimenopausal symptoms of night sweats and mood swings. Current medical problems include hypertension controlled with medications. Past medical history includes childhood illnesses of measles, chickenpox, and mumps. Family history is positive for diabetes on the maternal side and hypertension on the paternal side. Her only medication is an angiotensin-converting enzyme inhibitor and diuretic combination for hypertension control. She generally does not like taking medication and does not take any other OTC products.

Diagnosis: InsomnIa

1. List specific goals of therapy for S.H.

2. What drug therapy would you prescribe? Why?

3. What are the parameters for monitoring the success of the therapy?

4. Discuss specific patient education based on the prescribed therapy

5. List one or two adverse reactions for the selected agent that would cause you to change therapy.

6. What would be the choice for second-line therapy?

7. What OTC and/or alternative medicines might be appropriate for this patient?

8. What dietary and lifestyle changes might you recommend?

9. Describe one or two drug–drug or drug–food interactions for the selected agent.

Task 9 Final Project

You have chosen to join WHO. They are particularly interested in your microbiology skills since there have been many outbreaks of disease around the world. Now that you have reviewed your microbiology, you have been notified that you will be sent to one of two areas, West Africa, or the Middle East. In preparation, the Peace Corp would like you to choose one of these areas and complete a plan to deal with outbreaks or potential outbreaks in the area.

Choose an outbreak from those below to investigate.

Ebola or Middle East respiratory syndrome coronavirus (MERS-CoV)

Locate resources that will provide information about the infection, causing microbe, mode of infection, mode of spread, group or groups specifically susceptible, where outbreak occurred and when, clinical evaluation and diagnosis, infection control.

Find first person reports such as interviews or personal reports or journals on the outbreak. If you need help work with the librarian.

Design and submit an outline of the information you have uncovered. You will use this information to complete the project. The outline should contain information and cited resources for:

  1. Name of the disease chosen
  2. Causing microbe
  3. Mode of infection
  4. Mode of spread
  5. Group or groups specifically susceptible
  6. Where outbreak occurred and when
  7. Clinical evaluation and diagnosis
  8. Infection control.

Lesson 9

Activity 1: Completing the Project

First, using the information found, develop a one to two page maximum overview of the disease outbreak and the causal microbe.

Second, design a map showing the origin and spread of the infection.

Third, design a guide for communities on surveillance, control, preparedness and response to the outbreak. If this includes quarantine, describe how the community would carry this out, what resources would be needed, and what the communication protocol would be.

Submit completed project to Moodle

Activity 2: Oral Presentation

Prepare a 2 minute maximum presentation of your research to a WHO committee. You may do this presentation in person, live virtually on Adobe Connect, or video taped and submitted in Moodle.

Choose whatever you are comfortable with to write about. Be specific.

Here is the assignment:

Introduction:

Create a proposal for a piece of original and meaningful research based upon the following research prompt. Please note that we are not writing a research paper. We are writing a research proposal.

Prompt:

Choose three of our readings from this semester as well as one recent global event of significance (no, it doesn’t have to be COVID-19). Conduct a comprehensive critical analysis using your 3 selected readings as well as at least 10 other sources, ultimately demonstrating how your 3 selected texts clarify or illuminate the recent event about which you are writing.

Requirements:

The proposal project should include the following 3 sections:

Introduction:

This section should be a 250 word (minimum) description of your proposed research topic, including your hypothesis. Why is your research important? How does it expand/extend current research? What do you suspect the outcome of your research to be?

Methodology:

This section will be brief, likely one page only (250 words, perhaps even less), and will detail your approach to this topic. What types of sources will you seek? Why is your approach the most appropriate way to go about your research?

Annotated works cited:

This will be a list of potential sources for your proposed research. The list should be at least 13 sources long. Your 3 selected texts will be among these 13 sources. In other words, you only need to find 10 additional sources, which need not all be from our periodical databases. For each works cited entry, you should include a brief paragraph (3+ sentences) summarizing the source and another paragraph (3+ sentences) explaining how you expect the source to be useful to your research.

This alternative assignment definitely entails a good deal of work, but I believe you will find it fun as well as a nice break from the usual drudgery of writing papers (although I enjoy writing papers).
