Question Description: Throughout your practicum experience, you have been designing and implementing a project. You will now assess that experience. Prior to beginning this paper, discuss the topics below with your preceptor. Include their feedback as well as your own. In a 1,250-1,500 word document, discuss the following topics:

What was the focus of your leadership project, the goals you set for yourself, and the AONE competencies you hoped to achieve? The focus of this project was to measure quality data for the Hispanic population in the area of diabetes to see if they met quality goals and if not the reason why they did not meet goals. The feedback from nurses for patients who did not meet goals based on survey results of nursing staff was the main two reasons goals were not met were education/lack of understanding disease process and prevention and financial/insurance reasons. How did you discover the need for this project, and how did it fit into the organization’s philosophy and mission? Discussion of this project came during a quality meeting with the whole Waukesha/Jefferson market. It was discussed as to why the Hispanic population is not meeting their diabetic quality goals. Such as an A1C under 8%. The smaller market of Waukesha/Jefferson counties had better percentage of goals met-23.6% of patients did not meet goals, whereas the Milwaukee county market that is much larger had 33.1% of patients who did not meet goal.
Did you encounter any real or potential barriers during your practicum experience related to your project? It was hard to obtain the needed data to gather statistics about these specific markets. It was also hard to receive the nursing surveys that were created to gather data as to why patients are not meeting their quality goals for their diabetes.
Discuss how the outcomes of your project will be measured? Is your project one that is sustainable or is it designed to be a single occurrence? The outcomes can be measured by continuing to track the A1C quality goals and see if they are improving when the recommendations are implemented. This project can be sustainable if all staff actively participates.
Evaluate the success of your project. If you were not able to implement your project, discuss why. What are the next steps for the project? The project has not yet been implemented, the goal is to see if education for the disease project and preventive measures can be printed in Spanish so that patients can read and learn about everything they need to know in their native language. Also to gather resources for patients that could aid them financially such as insurance for low income patients, provide them with the Wisconsin Impact information so they can access all the resources available to low income families in their community.
