In addition, care coordination leaders must be confident in their abilities to navigate and lead change in their work environments. This assessment provides an opportunity for you to formulate a care coordination project planning strategy, develop a care coordination project plan for a selected population, and garner support for your plan from decision makers.Note: Your work in subsequent assessments will be based on the project plan you develop in this assessment.
